Person‐specific evidence was developed as a grounded theory by analyzing 20 selected case descriptions from interventions using the guided self‐determination method with people with various long‐term health conditions. It explains the mechanisms of mobilizing relational capacity by including person‐specific evidence in shared decision‐making. Person‐specific self‐insight was the first step, achieved as individuals completed reflection sheets enabling them to clarify their personal values and identify actions or omissions related to self‐management challenges. This step paved the way for sharing these insights and (...) challenges in a relationship with a supportive health professional, who could then rely on person‐specific evidence instead of assumptions or a narrow disease perspective for shared decision‐making. Trust in the evidence encouraged the supportive health professional to transfer it to the interdisciplinary team. Person‐specific evidence then enhanced the ability of team members to apply general evidence in a meaningful way. The increased openness achieved by individuals through these steps enabled them to eventually share their new self‐insights in daily life with other people, decreasing loneliness they experienced in self‐management. Relational capacity, the core of the theory, is mobilized in both people with long‐term health conditions and healthcare professionals. Further research on person‐specific evidence and relational capacity in healthcare is recommended. (shrink)

Parmi les travailleurs handicapés, plusieurs ont des emplois à temps partiel. Cependant, la recherche sur la relation entre handicap et travail à temps partiel n’a que partiellement éclaircit la façon dont les personnes handicapées qui reçoivent une pension d’invalidité complète ou partielle perçoivent et gèrent l’articulation entre travail à temps partiel et leur pension. Cet article s’intéresse aux stratégies employées par les personnes handicapées, tel qu’elles le racontent, pour gérer cette articulation. Le travail à temps partiel, utilisant les données d’entretiens (...) avec 10 citoyens norvégiens sont mobilisées. Les résultats montrent que la flexibilité et la visibilité sont des stratégies spécifiques que nos informateurs mettent en place quand les problèmes de santé et les heures de travail réduites ont comme résultat une connexion affaiblie avec le lieu de travail. L’article montre la façon dont le discours autour de l’emploi payé facilite ces stratégies. L’article se conclue sur des recommandations relatives aux conditions à mettre en place pour accompagner l’articulation entre travail à temps partiel et pension d’invalidité partielle. (shrink)

To be free is to escape all limitations and obstacles—or so we think at first. But if we probe further, we discover that freedom embraces its own necessities, a set of conditions without which it could not exist. _Freedom's Embrace_ explores these necessities of freedom. J. Melvin Woody surveys competing conceptions of freedom and traces debates about the nature and reality of freedom to confusions about knowledge, humanity, and nature that are rooted in some of the most fundamental assumptions of (...) modern Western thought. The preemption of freedom as an exclusively human privilege with all nature relegated to mechanical necessity is a fatal error that renders both humanity and nature equally unintelligible. What distinguishes human beings from other animals is not freedom but the use of symbols, which vastly extends the range of available options and enables us to envision freedom as an ideal by which customary institutions and norms may be judged and transformed. By carefully surveying its necessary conditions and limitations, Woody reconciles the salient competing conceptions of freedom and weaves them together into a richer and broader theory that resolves old controversies and opens the way toward an ethics of freedom that can meet the challenges of relativism and nihilism that arise from recognizing the historicity and malleability of culture. Article 3.4 of EC directive 89/381 requires member states to take "all necessary measures to promote Community self-sufficiency in human blood or human plasma" and, for this purpose, to "encourage the voluntary unpaid donation of blood and plasma". This paper presents an ethical case in support of the policy of voluntary, unpaid donation.

We consider the following question of Kunen: Does Con(ZFC + ∃M a transitive inner model and a non-trivial elementary embedding j: M $\longrightarrow$ V) imply Con (ZFC + ∃ a measurable cardinal)? We use core model theory to investigate consequences of the existence of such a j: M → V. We prove, amongst other things, the existence of such an embedding implies that the core model K is a model of "there exists a proper class of almost Ramsey cardinals". Conversely, (...) if On is Ramsey, then such a j, M are definable. We construe this as a negative answer to the question above. We consider further the consequences of strengthening the closure assumption on j to having various classes of fixed points. (shrink)

Shared decision-making involves health professionals and patients/clients working together to achieve true person-centred health care. However, this goal is infrequently realized, and most barriers are unknown. Discussion between philosophers, clinicians, and researchers can assist in confronting the epistemic and moral basis of health care, with benefits to all. The aim of this paper is to describe what shared decision-making is, discuss its necessary conditions, and develop a definition that can be used in practice to support excellence in maternity care. Discussion (...) between the authors, with backgrounds in philosophy, clinical maternity care, health care management, and maternity care research, assisted the team to confront established norms in maternity care and challenge the epistemic and moral basis of decision-making for caesarean section. The team concluded that shared decision-making must start in pregnancy and continue throughout labour and birth, with equality in discourse facilitated by the clinician. Clinicians have a duty of care for the adequacy of women's knowledge, which can only be fulfilled when relevant knowledge is offered freely and when personal beliefs and biases that may impinge on decision-making (defeaters) are disclosed. Informed consent is not shared decision-making. Key barriers include existing cultural norms of “the doctor knows best” and “patient acquiescence” that prevent defeaters being acknowledged and discussed and can lead to legal challenges, overuse of medical intervention and, in some areas, obstetric violence. Shared decision-making in maternity care can thus be defined as an enquiry by clinician and expectant woman aimed at deciding upon a course of care or none, which takes the form of a dialogue within which the clinician fulfils their duty of care to the client's knowledge by making available their complete knowledge (based on all types of evidence) and expertise, including an exposition of any relevant and recognized potential defeaters. In this paper, we describe Blackboard’s Online Journal program and explain how we use the online journal in a variety of philosophy courses. We outline our pedagogical motivation for using online journals and analyze how online journals help to improve our students’ ability to read, write and think philosophically. We analyze the strengths and weaknesses of online journals in comparison to online discussion boards. Finally, we address several concerns that philosophy teachers may have about using online journals.

Amit's Hebbian model risks being overexplanatory, since it does not depend on specific physiological modelling of cortical ANNs, but concentrates on those phenomena which are modelled by a large class of ANNs. While offering a strong demonstration of the presence of Hebb's “cell assemblies,” it does not offer an equal account of Hebb's “phase sequence” concept.

Anselm's argument has two distinct conclusions: (a) we cannot intelligibly doubt that god exists, and (b) this god, whose existence we cannot doubt, exists necessarily. if we replace anselm's vague conception of god by the spinozistic conception of substance, a defensible version of the ontological argument, understood as having these two conclusions, can be constructed. two important consequences of this analysis are: (1) the ontological argument, properly understood, deals simply with the concept of substance. it is a further question whether (...) substance ought to be understood as god. (2) the ontological argument does not actually demonstrate the existence of anything. it simply makes clear the consequences of adopting a metaphysical view which employs the spinozistic conception of substance. A central argument for the view that God's necessary omniscience [( Bgf p )] precludes freewill is unsound, because the necessity of the consequence is not the necessity of the consequent, and nor is Bgf true. God's belief in some particular proposition f about what I will do is not necessary, as I might do something that makes ~ f true. Fischer and Tognazzini claim that this counterargument argument assumes that I must freely do the something that makes f true. (...) But plainly it doesn't. All that it assumes is that I will do the something that makes f true. It makes no difference whether I do that thing unfreely, or "deterministicall". The argument does not assume the existence of a case of freewill in the face of divine foreknowledge, but instead considers the necessity of God's belief in some particular designated proposition f. The argument does not depend, as Fischer and Tognazzini suppose, on whether God's knowledge is a function of the facts (omniscience) or vice versa (infallibility). (shrink)
